Welcome to Integrative Medicine at Crossroads! We look forward to a long relationship and to providing you with outstanding traditional and complementary health care.

We want your first visit to be as informative, comfortable and convenient as possible. To help us make your visit the most positive experience it can be, we have compiled the following helpful hints to assist you. Remembering these items will help our office and you when visiting us for medical care.  Please bring with you, or be prepared with:

  • Your past medical history
  • Over-the-counter medications (when possible, bring the actual medications)
  • A list of (or the actual) prescription medications you are currently taking and the dosage instructions
  • Insurance information/card.  Please confirm with us that we accept your insurance coverage prior to your visit.


Most HMO’s require a referral to see any doctor other than your primary care physician. If you do not obtain a referral for a visit with a specialist, you may not be seen. Please be aware that referrals often require authorization from your insurance company. For this reason, Integrative Medicine at Crossroads asks that you allow our referral coordinator at least three business days to process all referrals.

Note: Integrative Medicine at Crossroads does not “back date” referrals.

Please note the following important information about referrals:

It takes three days for a referral to be complete, unless it needs to be authorized from the insurance company; then it may take longer.
We do not fax referrals. It is the patient’s responsibility to get the referral to the specialist’s office.
Referrals need to be picked up between 8:30 a.m. and 4:00 p.m., unless other arrangements have been made.
Please note: the referral line voicemail does indicate how long a referral takes to be processed, the hours of referral operation and that we do not fax referrals.
For procedures and surgeries, the specialist needs to request the referral and we will in turn send the referral directly to them.
